Meet with your Teaching Major Academic Advisor to start the Advisor Approval Form. 

Admission Requirements

If applicable, meet with your Teaching Minor Academic Advisor(s) to fill out the Advisor Approval Form. 

It is highly encouraged to include a minor with your major. You will also be licensed to teach with any minor you add onto your major. 
NOTE: Non-Teaching Minors do not need to be listed and do not need a signature. 

If Not Current, Complete a Background Check

You will need to have a background check completed prior to your application for your Educator License. Background checks are valid for a period of five years before licensure.

If you hold a current teaching license, you will not have to complete another background check. We cannot accept copies of background checks done for a school district or other employment. To apply for a background check, go to USIMS and sign in or sign up: usims.schools.utah.gov

Background Check Instructions

• Make an Account with USIMS (Utah State Board of Education)
• In USIMS, go to Personal Information, and mark "Utah State University" as your Utah Teacher Education Program
  • Select Personal Information from the Task List
  • Select that you are enrolled in a Teacher Education Program, and select the University that you are attending and press the Save Button. 
• Sign up for a Livescan Fingerprinting with USU Police (Logan Campus) 
  • Statewide Livescan Locations
• Bring your CACTUS ID to the Livescan Appointment (found in Personal Information in USIMS) 
• NOTE-Background checks can take up to 6 weeks to clear

• Screenshot of ELED/ECED Email Listserv confirmation
• Screenshot of FBI/BCI Teacher Background Check Clearance Date in USIMS
• Screenshot of marking "Utah State University" as your Utah Teacher Education Program (Beginning Fall 2026)
  • Login to to USIMS
  • Select Personal Information from the Task List
  • Select that you are enrolled in a Teacher Education Program, and select the University that you are attending and press the Save Button.
• Click "UPLOAD" for each document AFTER you upload them.
